Desmoplastic mesothelioma

93 years old man with mediastinal and pleural tumor.
Spindle cells with storiform growth pattern. A dense collagen fibrosis with atypical tumor cells is characteristic of desmoplastic mesothelioma.
Immunhistochemical postitivity of tumor cells for calretinin, D2-40, CK19 and WT-1. Differential diagnosis to sclerosing mediastinitis, organizing pleuritis and solitary fibrous tumor may be difficult. Important is the positivity for cytokeratin of the invasive cells, high proliferative activity (ki-67), cytologic atypia, loss of Desmin- and partially Vimentin-expression, Expression of GLUT1 and Mel-CAM (CD146, in normal mesothelial cells not expressed). FISH: Loss of genetic material on the 9p21 (p16 locus). Clinical presentation is similar to other mesothelioma types.
