MGUS IgG Kappa, metastatic melanoma of the uvea. Infiltration of the bone marrow by melanoma?
Hypercellular bone marrow with an irregular distribution of megacaryocytes without cytologic atypia. Interstitial expansion of plasma cells with Russell bodies. CD38 confirms the plasma cell expansion (infiltration volume 7-8%). Light chain immunohistochemistry confirms a kappa light chain restriction (kappa:lambda ratio 7-10:1). No infiltration by the known melanoma.
